The Transitional Housing Program is designed to provide affordable housing and case management to homeless families. Participants can stay in the Program for two years while working towards self-sufficiency.

Participants must complete a Family Action Plan that outlines family goals that is designed to improve their ability to maintain stable housing. The three main areas of the Plan are: Residential Stability which focuses on obtaining permanent housing; increased skills and Income which includes job training, obtaining GED, job training, and classes for job seekers; and Greater Self Determination which focuses on case management, and completion of the Family Action Plan.

Families must meet with a case manager at least two times per month and all participants are reviewed and continuation in the Program is based on effort they put into meeting their goals.

The families receive assistance with rent, guidance in setting and meeting goals, workshops in basic life skills, parenting classes, and tenant education.
